New Patient Registration

If you live within our practice area you may register at reception by filling in a form and providing two forms of ID including proof of address.

If you are accepted you will be asked to attend a registration appointment with a doctor or nurse.

More information about how to register with a GP can be found on the following website:

NHS Choices Information on Registering with a GP

Change of Personal Details

Please notify the practice as soon as you can of any change of address or telephone number, so that your records can be kept up to date to ensure that reminders for blood tests, health checks, smears etc. go to your correct address. It is also important that we have your correct address and telephone number should the doctor need to make a home visit.

If you move out of the practice area the doctors can no longer be responsible for your health needs. We therefore advise you to register with a surgery in the area you live as soon as possible. Please ask at reception for further details.

Guide to GP Services

The Royal College of General Practitioners has produced a useful guide for patients about the services on offer at GP Surgeries and how to access them.  You can download the guide below.
